How To Apply
We seek to serve 12-18 year olds who struggle with emotional, spiritual, or behavioral issues. We accept applicants based on need and appropriateness for our program. To apply is as easy as 1, 2, 3!
1. Fill out the Participant Application Form. We recommend this form is filled out by a parent or guardian. The form must be completed in full. You may print the form below and return it to 162 Russell Cave Rd. Georgetown, Ky 40324 or firstname.lastname@example.org. You can also click the quick and easy link below to apply online.
2. Provide SHEM with the name of a counselor, psychiatrist, mentor, doctor, or family friend who as spent a significant amount of time with the participant and can speak on behalf of their overall well being and history. If a release must be signed for confidentiality, please fill out a release of information form at the designated office. If you are a reference for participant, click one of the links below:
3. After the participant registration form and participant referral form have been received we will schedule you for a Meet and Greet visit out to the farm to meet the staff and horses. This is very informal and will be a time to become acquainted and voice any questions or concerns. This time will be no more than one hour and may include working with a horse so please dress with closed toes and comfortable clothes! Our only mandatory requirements for all participants is that they willingly choose to be at the farm and participate in the program. During this time, this question will be addressed and a self-assessment survey will be filled out by the participant.
